Report Forms

The duties of a match official are not over once the final whistle has been blown. As referee or a touch judge, it is important the relevant paper work is completed, especially if a player has been dismissed from the field.

Team sheets should be filled in and signed accordingly, while dismissal forms, brawl reports and abuse forms should be completed within three days of the match's conclusion.

Below are copies of ALL the appropriate and relevant forms that match officials from Warrington RLRS should need. It is important the correct form is filled in and sent to the correct disciplinary representative.

Should any match official need help in completing a report form, they should contact their Development Officer.
